A new deal has been agreed to help provide care to frail people in their homes in Caerphilly county.
The partnership agreed between five councils and the health board will mean frail people will be treated at home rather than in hospital or care homes.
Caerphilly County Borough Council, which last week agreed to join the Gwent Frailty Programme, said it meant people could receive support with dignity.
Councillor Judith Pritchard, Cabinet Member for Social Services said: “The Gwent Frailty Programme is excellent in that it will bring together staff from health and social services to provide a seamless service to those residents who do not need urgent treatment in hospital.”
