A health board has offered an “unequivocal apology” to the family of a Bargoed man who bled to death at the Royal Glamorgan Hospital in Llantrisant.
An inquest into the death of 47-year-old Myron Hall was told how the cancer patient had bled to death after a breathing tube was incorrectly inserted into his neck.
The coroner, recording a narrative verdict, said there had been a series of errors leading to Mr Hall’s death in 2007
In a statement Cwm Taf Health Board said it offered an unequivocal apology and that the death was “premature and avoidable” and should not had happened.
It added that it will again be reviewing its ENT cancer services.
